So much great care happens in the doctor’s office. But what happens after that appointment ends? At St. Joe’s, we know that there are folks in our community struggling with issues that are not necessarily in their medical records but, left unchecked, can have a direct impact on their health and well-being.
Our care management team works to connect with our most vulnerable community members; the elderly, those who suffer from substance use disorder, or just anyone who is struggling with food insecurity, transportation issues, medication affordability, and anything else that prevents them from their best health. Our goal is to help our community members feel supported through care management services in the hospital, the emergency room, primary care offices, and wherever else they may be in our healthcare system. At St. Joe’s, we embrace our mission to care for the community by providing people with the support they need, when they need it and where they need it. That’s community medicine.
